When I started saying this is in late 2019, it wasn’t a foreign concept to the sports medicine community. I didn’t invent the wheel. Much to my dismay, it was a new concept to many fantasy gamers and analysts alike. The term “injury prone” (“IP” moving forward) was and continues to be thrown around consistently in fantasy circles and is used to describe any player who misses even a quarter of game action.
The goal here is to first give fantasy gamers and bettors a clear understanding of how to view injury histories from a big-picture perspective and debunk the reductionist mindset of “injury prone.” Then later in the book, I’ll present my Injury Volatility Tiers, which is how I quantify and categorize injuries with expected future impact. This is meant to be an evergreen chapter outlining the basics of injury analysis to help gain the upper hand on trade partners, snipe leaguemates on draft day, and be an objectively more accurate fantasy football prognosticator.
What I’m Not Saying
Not Saying: Exceptions Don’t Exist/Patterns Don’t Exist
There are specific cases in which it’s reasonable to question whether a player is at a higher predisposition for injuries compared to counterparts based on genetic variability and/or medical history (formerly James Conner and currently Joe Burrow). However, to believe that every player falls either into the immortal Frank Gore category or the less-fortunate Burrow category is a fallacy. Oftentimes players like Burrow and Gore are used as standards when in reality, they’re exceptions.
The Bottom Line: Making assumptions based on coincidental injuries and completely ignoring injury histories is suboptimal.
Not Saying: Injury Histories are Irrelevant
Usually IP pertains to a player with a minimal injury history or a player who has a completely random history of injuries. (James Cook and Joe Burrow, let’s update names) fall into the latter category. The bottom line is that when a player has a hand fracture followed by an MCL tear then later on is concussed, there’s no statistical or scientific pattern to be drawn. That’s bad luck. That’s the game of football.
The opposite is also true: a player with a clean bill of health does not make him impervious to contact injuries like ACL tears, MCL sprains and ankle sprains. The perfect example is Jonathan Taylor, who allegedly hadn’t even missed a practice since high school before the 2022 season.
So then when and how do injury histories matter? When viewed with the appropriate context. In fact, injury history is the best tool available to fantasy players and sports bettors. As it turns out, more games missed in college generally leads to more games missed in the NFL.
Previous injury is the best predictor for future injury.
And Another Thing
Here’s the thing about “compensation injuries.” Elite athletes won’t develop these issues overnight. There’s simply no good evidence that demonstrates [X] injury leads to [Y] “compensation injury.” The type of secondary injury that does exist has to do with the 3-6 times increase in soft tissue injury risk when players alter physical workloads in a meaningful way.
An example of a player who may have sustained a quad strain due to the up-and-down nature of practice-game-rehab is Christian McCaffrey in 2020 and 2021. Which is why I was bullish on him staying healthy in 2022. He was a lesson in patience.
When a player sustains an ankle injury here’s what it tells us: that player sustained an ankle injury and he’s at risk for another - wait for it - ankle injury (on the same ankle). Previous injuries automatically increase a player’s future reinjury risk for that specific issue, not another random unrelated ailment.
The Bottom Line: On draft day, the best way to know when an injury history matters is to follow an injury analyst you trust.
Not Saying: Injuries Don’t Impact Performance
Know which injuries are the most detrimental. It’s also important to keep proper context such as draft capital, age and severity of injury. These help predict if a player will perform to pre-injury levels on the field following an injury. Broadly speaking, players will bounce back from significant injuries if the following criteria are present:
< or = 25 years old
NFL draft capital in the 4th round or higher (the higher, generally, the better)
Studies indicating the injury is not catastrophic (tendon ruptures)
Organizations typically give a much longer leash to players they invested high draft capital on. Whether that’s illogical or not, it tends to be true.
Furthermore, these athletes tend to have a better “athletic reserve” following injuries and their rehabs tend to go more smoothly.
The Bottom Line: Generally speaking, tendon tears and concussions will impact most players across the board equally. After that, look to take advantage of your leaguemates who fade young players with high draft capital and relatively benign injury histories.
What I am Saying
Am Saying: 2.3% of Games are “Injury Free”
I cite this statistic often and it’s really quite that simple. Being injured in the NFL is the norm and health is the anomaly. To blindly assign the term IP to a player without any real investigation is statistically unlikely to be true. Furthermore, not every injury is created equally. If a player misses games because of a collarbone fracture, ACL tear, then a ruptured kidney, it’s self-sabotage to fade that player due to injury concerns. Ask Keenan Allen, who is the proud owner of that very history. Allen wasn’t injury prone. He was prone to playing NFL football. FYI, Allen played 78 of the next 81 games for the Chargers after these injuries.
Am Saying: Style of Play is Virtually Irrelevant
There’s no objective definition to determine “style” in the first place. That means there’s no standard for the phrase itself. Football is a violent game. No standard means no precedent, which makes it a tenuous argument against players who are injured. For example, it’s fair to say George Kittle has missed more games than average due to injury. It’s not fair to say he’s missed those games due to “style of play.” As this thread points out here, his significant injuries have come as a defenseless receiver or blocker.
There’s data to show that running backs (who presumably are IP) aren’t actually injured more frequently than other skill players. Instead, they’re more likely to suffer different categories of injuries that require more healing time and the positional scarcity makes them more memorable.
However, pass catchers are actually more susceptible to concussions and ACL tears. These are the result of positional demands and something to consider.
Am Saying: BMI is Relative
As it relates to BMI, there’s no evidence indicating that there’s a difference in injury rates for running backs with a BMI between 29-32 kg/m^2 (5-foot-8 to 6-foot-1 and 207 lbs. to 243 lbs.). As long as a RB meets that threshold, there’s no reason to be concerned about injuries.
As for smaller RBs under 200 lbs., in theory, they are injured more frequently due to contact injuries, but the sample of these players is so small that it’s difficult to draw any sort of conclusions – even for De’Von Achane and Jahmyr Gibbs. These players should be analyzed on a case-by-case basis by weighing previous injury history. The same can be said for wide receivers. If a player doesn’t meet the BMI threshold for their given position, the chances of success, not injury, should be the major concern (even though DeVonta Smith says hi).
The Bottom Line: There’s no real way to quantify “style of play” as it relates to injury risk. It’s purely a narrative. If a player reaches the threshold for BMI at their respective position, there’s no reason to be concerned about their health.
Player injuries, with appropriate analysis, create a wealth of opportunities to steal players from other nervous managers. However, it’s much easier to calculate a static injury risk quotient than it is to adjust for a player’s physical volatility mid-season. So, it’s important to know timelines and rehab progressions for the most common injuries to exploit market inefficiencies.
Below is a list of injuries that are mostly unpredictable, but can provide managers with some sweet, sweet fantasy football equity during the season. The tables below contain “Seasonal Recurrence,” which is how often an injury can carry over into the next season. Meanwhile “Weekly Recurrence” is how often an injury lingers throughout the season.

Summary
There’s no one right way to view player injuries but there is a wrong way. At its core, injury analysis refines the consensus’ idea of what a player’s floor/ceiling really is. The wrong way to assess these situations is to blindly label players IP. This leads to value leaks across multiple leagues and is suboptimal. For an edge on the competition always remember:
Injury prediction is impossible. Instead, stay curious and be on the lookout to exploit leaguemates who are quick to write off a player due to injury concerns.

Worded differently, from 2013 to 2022, there was ~36% chance that the RB drafted in the 1st round would miss zero time due to injury. For the WRs drafted in the top 12, that number was 47%. The rates are much tighter than the average drafter realizes.

Things get messy for top-2 RBs by ADP vs. top-2 WRs by ADP. The amount of missed games due to injuries for WRs is 1.8 vs. 4.8 for RBs. All of this data indicates that RBs aren’t necessarily injured more frequently than WRs, but RBs are more likely to sustain severe injuries due to contact.
A major factor that makes games missed by RBs stand out is the scarcity at the position as RBBCs (running back by committee) have taken the league by storm. Case and point: 2023 was the first time since the turn of the century that no running back in the league carried the ball 300 times.
As a result, fantasy RBs who make a legitimate impact are few and far between. Meanwhile guys like Danny Amendola helped you win a DFS tournament at his near-min salary so you could scrape two of the top five players on the slate. This brings up the concept of a RB handcuff, who is a pure lottery ticket vs. one who, based on their teammate’s injury history, has a chance to provide spike weeks and/or take over a backfield.
Handcuffing for Injury Upside
TL:DR
Rarely should a fantasy manager handcuff their own starting RB
Use the IR slot for players who are eligible and immediately stash another player for upside (in this exercise, a handcuff RB)
Handcuffs can provide legit league-winning value to rosters and is the basis of “Zero RB”

I’ll say it again - you can see that most of these guys don’t hit. Williams was obviously an outlier but having multiple irons in the fire improves your odds of hitting. Remember from last year guys like Charbonnet, Warren and Spears were “just” ADP beaters but the logic remains: they had a highly attainable ADP lowering opportunity cost and probabilistically had league-winning upside. The fact that catastrophic injury didn’t hit doesn’t make the process wrong!

QB injuries were a hop topic in 2023. But were there really more injuries than usual? Yes and no.
Even though the number of starting quarterbacks who missed time due to injury was nearly identical to last season – 20 QBs vs. 19 in 2022, respectively – passers this season missed a whopping 102 total games.
In 2022, starting quarterbacks missed 88 games. That means there was a 15.9% increase year-over-year in total games missed among starting signal-callers.
Joe Burrow (calf/thumb), Kirk Cousins (Achilles), Daniel Jones (neck + ACL), Anthony Richardson (shoulder), Deshaun Watson (shoulder), and Aaron Rodgers (Achilles) combined to miss a total of 67 games on their own. Woof.
The Impact of QB Injuries
The downstream effects are just as jarring. It seems obvious, but bears repeating that highly drafted skill players were also impacted.
Among the teammates of QBs who missed at least seven games, these skill players were drafted in the top 6 rounds per August Underdog ADP:

There are plenty of knits to be picked with some of these players. And some of them had injuries of their own. But the point is that several offenses had to endure tectonic shifts in game planning and overall infrastructure.

So how did this impact the backfields and pass catchers? To answer that question I used the incredible FantasyPoints data suite (seriously, if you can swing it, this is the best data package in the game). First, every team who had a starting QB go down with injury in 2023 were identified. Then, team backfield fantasy points per game and team pass catcher points per game respectively were calculated pre and post QB injury. For example the average amount of fantasy points per game that Colts’ pass catchers with and without Anthony Richardson were calculated. The same was done for the Colts’ backfield. Then the average post-injury was divided by the average pre-injury. The exact same was done for backfields. Backfield example:

Results
Below are the raw numbers for how pass catchers and backfields fared. The percentage represents how much a team improved or worsened based on when the starting QB was active by calculating the difference in fantasy production.
For example, the first number represents that the Chargers saw a (-) 21% dip in fantasy points from pass catchers without Justin Herbert active. That was the worst in the league, by the way.

Pass Catchers
Overall, 6 of the 12 teams’ pass catchers worsened in fantasy points per game. Those teams were:

Chicago is a great place to start as an example as to why this exercise is imperfect on the surface. Overall, the Bears’ pass catchers only dropped (-) 2% in overall production. However, DJ Moore averaged 20 points per game with Justin Fields (which included a 49 and 27 point explosion).
With Tyson Bagent under center, Moore averaged fewer than 10 points per game - a drop of more than (-) 50%. The takeaway is that Moore’s ceiling and floor were negatively impacted after Fields was injured. The flip side is that Cole Kmet’s average with Bagent jumped from 11 to 13. Additionally, 2 of Kmet’s 3 best fantasy outings came with Bagent at QB. The real kicker? The Bears' pass catcher ppg was unchanged as a whole! This is the perfect example of how a change at QB impacts players’ floor/ceiling - for better and worse depending on the system.
Another case to look at is Davante Adams with the Raiders. He went from 15 ppg with Jimmy G to 16 ppg. Adams is simply built differently. The Raiders’ offensive ecosystem rightfully revolved around him regardless of who was under center. So, even though Adams’ production was technically “better” the Raiders’ pass catchers overall saw a negligible 6% decrease overall.
So if a true generational alpha WR sees his QB miss time, the offense shouldn’t skip a beat, right? Not so much. In 9 games with Joe Burrow, Ja’Marr Chase surpassed 12 fantasy points (sem-arbitrary) 66% of the time. Without Burrow, he crossed that 12 point mark just once or 20% of the time.
Next, let’s dig into Justin Jefferson who, for all intents and purposes, was healthy by the time he returned from his hamstring injury. The result? JJ went from the WR3 averaging 22 ppg all the way to WR21 (that’s twenty-one) averaging 14 ppg. Even though a guy like Jefferson can go off at any time (see Week 18) and even though we’ll take 14 points any day of the week, without Kirk Cousins he didn't pay off anywhere near his 1.01 ADP. Unsurprisingly, the Vikings’ fantasy ppg from pass catchers dipped by (-) 19%.
Lastly, looking at the data for teams whose pass catchers only improved (so excluding teams who were worse), we see that overall that improvement was modest at (+) 11% and was bolstered by a single outlier. This will come into play while looking at the backfields.
Backfields
Here are the teams whose backfields worsened in fantasy points per game. Again, some of these were small samples so keep that in mind:

If you’re wondering, the only 2 teams who worsened in both backfield and pass catcher ppg were the Chargers and Texans.
Overall, backfield fantasy points per game improved by (+) 14% across the board. The improvement amongst teams who did get better was high at (+) 49%. This stands to reason: when the starting QB goes down and a team can #EstablishIt even a little, they will. This stat passes the eye test when looking at Pittsburgh, Chicago and New York (Giants).
When it comes to team management while dealing with real life NFL QB injuries, it pays to roster the average to good RB. To further this point, look at Joe Mixon after the Burrow injury. After Burrow went down, Mixon averaged 18 ppg. Before, Mixon averaged just 13 points per game. The Bengals saw a commensurate (+) 70%(!) increase in backfield production.
Another example: in games without Jimmy G, Josh Jacobs’ average fantasy points per game were 16 and 17 respectively as compared to 13 and 11 with Jimmy G. Now, theoretically Jimmy G was just benched but philosophically, the point remains. When the Raiders decided to establish it, Jacobs fantasy managers reaped the rewards.
Takeaway
Overall, there were more QBs in 2023 than usual who missed ~40% or more of the season despite “minor” injury rates being relatively static. The skill positions were impacted in opposite and generally expected ways. When a starting QB was injured, team pass catchers, even the alphas, were generally more volatile in fantasy scoring. When it comes to team management while dealing with real life NFL QB injuries:
Rostering the average to good RB1 of a team backfield is more likely to pay off than a pass catcher
As for pass catchers who lose their starting QB, ceiling games are less likely to come by as evidenced by guys like DJ Moore, Justin Jefferson and Ja’Marr Chase.
Overall, these results make sense: Any “improvements” in receiving points per game we observed this year were modest and in reality much less predictable. The dude throwing the ball is going to directly impact the dudes catching it. The backfields, however, are generally more predictable and should be targeted.
ACL Management
TL:DR

Medical providers consider hundreds of factors while rehabbing an ACL tear. For fantasy purposes, here are the 2 most important ones:

The average return to sport time for players coming off an ACL tear in the NFL is approximately 11-12 months. However, many NFL athletes return and perform between 8-10 months, such as Chris Godwin and Michael Gallup. Most surgeons won’t completely clear a player for game action until at least 9 months, as retear rates at 9 months are no different than at 12 months.
In contrast, returning before 9 months has been documented to increase risk of retear rates seven-fold in kids. Now, grown men in the NFL are not kids – but the basic principles of biology and healing still apply.
Function
Returning to the field, even if a player feels and seems ready to go, could result in workload-related injuries like Chris Godwin or under-performance like with Michael Gallup.
Returning to play is not the same thing as returning to perform.
Running Backs
The Harsh Reality
Since 2010, a total of 36 running backs have torn their ACL and attempted to return the following season. In this group, 29% of players did not return to the NFL and 6% took two or more years to return. This means that over one-third of the RBs who sustained an ACL injury either had their careers ended or dramatically altered.
This puts a damper on the “2nd year after ACL bounceback” narrative. This is also why the narrative that ACL tears are no longer a career-threatening injury is a myth. This injury very much still impacts the careers of RBs.
Identifying Outliers
After removing RBs who did not return immediately after surgery, we find that only five RBs reached at least 85% of pre-injury fantasy production levels and scored at least 10 PPR points.

These are the boxes these five check when looking for outliers who can bounce back in year 1 from ACL tears.
26 or younger at the time of injury

Relatively straightforward surgical procedures (generally speaking means no LCL or PCL involvement)
That criteria is based on a study analyzing 47 NFL players over the span of 27 years. What the authors found was that the following variables were independent predictors of returning to previous levels of performance/NFL longevity:
Age < 25 years old (previous performance)
Offensive players (performance)
NFL draft capital in rounds 1-4 (longevity)
Looks familiar, right? Ostensibly, the younger the player, the more talented/athletic. And the higher the draft capital, the better the chances of an immediate smash return. These players have fewer complications, more front office support and overall supreme talent. They can progress more quickly through rehab and are least impacted by ACL injuries. This study on NCAA players supports the argument for youth aiding in ACL rehab. Every step matters in the NFL and physiologically, the younger players are in their physical prime.
“Compensation Injuries”
Since 2008, 48% of RBs miss no time after ACL surgery. Additionally, 71% of those RBs who did miss time due to lower-body injuries either started the season on PUP and/or had complications due to the knee itself. Only 19% of the sample sustained a non-knee lower-body injury. Those RBs were identifiable in the preseason. Simply put: an ACL tear doesn’t preclude secondary injury. Here’s the sample just since 2012.

Receivers are slightly different from RBs as the specific successful archetype doesn’t necessarily require elite athleticism. Instead, receivers who have had a minimally complex injury are young, and interestingly average at least 80% success rate against zone coverage (shoutout to Reception Perception and Matt Harmon) have found themselves on this list. Those who didn’t meet the criteria of bouncing back had an average 75% success rate vs. zone. The reason for this success despite no freak athleticism is likely because WRs who don’t use physicality to win the first place (think players like Cooper Kupp and Julian Edelman) are extremely gifted in finding soft spots in the defense. Even if they “lose a step” in that first year back, their ability to “win” remains. Lastly, the average amount of games played among these outliers: 15. Further putting to rest the “compensation injury” argument.
Quarterbacks
In the modern era, big studies like this one have concluded QB play largely does not take a hit after an ACL tears. However, zooming into fantasy-relevant QBs paints a slightly different picture. It’s important to note this is a small sample and may not reflect returns overall. However it’s a decent place to start.
First, it is true that QBs take less time to bounce back as illustrated by Joe Burrow, who returned in 9 months and 1 week for an injury that would have taken any RB at least 10-11 months to rehab from. Similarly, Robert Griffin III returned in 8 months despite LCL involvement.
Generally speaking, though, QB rush attempts per game do take a hit in this sample. When excluding Deshaun Watson, the average decrease is by about 19%. This results in QBs averaging just 86% of pre-injury levels (fantasy points per game). But again, it’s important to note this sample was taken to compare QBs who were in this era and/or had relatively similar skill sets. A Tom Brady ACL tear from 2008 probably does us no good.

Overall, QBs can return to play at a faster rate but rushing attempts and fantasy points per game could take a hit. Again, the sample size is tiny – so please be careful to not extrapolate this data too far. Of course, the better the talent and situation, the better the outcome.

Any player who tears an ACL can return within 9-11 months (8-10 months for QBs) so long as the injury does not involve the LCL and/or PLC and/or additional damage
The younger the athlete, the higher the NFL draft capital, the better

I’m back at it again – here to ruffle the feathers of @runthedamnball69 and @girldad420 on Twitter.
My first publication on this topic suggested that rushing as a quarterback does not preclude higher injury rates compared to less mobile quarterbacks. Usually, I only briefly reference a link but reviewing the conclusions of that previous study is important, so here’s the TL;DR:
Nobody is positing that a quarterback should ram into a defender repeatedly because it’s safer than passing the ball.
From 2016 to 2020, thousands of plays were analyzed, and the study found that dropbacks ending in sacks and knockdowns/hits have the highest injury rates among NFL QBs compared to designed runs and scrambles.
Case studies like Robert Griffin III and Cam Newton are considered outliers, and the running style of quarterbacks like Lamar Jackson indicates that many of their rushes do not result in contact.
Examining the careers of historically “mobile” quarterbacks demonstrates their longevity and success.
The narrative of pocket passers being less injury-prone lacks evidence, and the trend is shifting toward recognizing the value of ultra-athletic quarterbacks in both the NFL and fantasy football alike.
Now, why would you need more evidence than that? Here’s what this new study aims to accomplish:
Find statistical significance and impact of the data
Further define “mobile” and “non-mobile” QB
Identify how many games QBs in each category missed
Account for BMI and age
While it's true that quarterbacks who take hits are at a higher risk of getting hurt, it's essential to understand that being a mobile quarterback does not necessarily mean facing a higher likelihood of injury. In fact, when quarterbacks are prepared to take hits or skillfully avoid them, their injury rates aren't as high as the narrative implies. The goal of this study is to find statistical data to support that hypothesis. Here’s a brief description of the population and methods.
Criteria and Definition
The study examined quarterbacks from 2018 to 2022 who played in at least three games and/or were projected to be the team’s starter that season prior to injury. Players like Jeff Driskel and Taysom Hill were removed from the sample. Sporadic spot starters were also excluded. There were a total of 195 QBs included in the sample.
A designed run includes quarterback sneaks and RPOs while excluding kneeldowns. Scrambles include any time a quarterback leaves the tackle box outside of the play design. Hits are any contact made on a quarterback by a defender while in the pocket that does not ultimately end in a sack (i.e. hit as thrown). Sacks are self-explanatory.
Statistics
None of the correlations run during this study were bigger than “small,” which is expected as football is a random game that is nearly impossible to predict. However, if a relationship is considered “significant,” that means that the data is giving a signal that is statistically unlikely to be random. The r^2 projects “how much” of the results (i.e. games missed) can be explained by the independent variable (i.e. sacks + hits). In that specific example, it’s estimated that 62% of missed games by QBs can be explained solely by hits + sacks.
Mobile QBs do not miss more games than non-mobile QBs
There were 62 total QBs who met the threshold of > 3 scrambles + designed rushing attempts per game. We’ll consider this the “mobile” group. The rest are considered the “immobile” group, as the per-game average in designed runs + scrambles was 2.

BMI, Age and Injury
QBs with a BMI of 28 or lower missed on average 1 game per season while those with a BMI above 28 missed an average of 2 games. This flies in the face that mass leads to durability. The theory here is that unless your name is Cam Newton or Anthony Richardson, the more mass a QB holds, the less mobile they are. The less mobile they are, the less time they have to prepare for contact. Really, this ultimately illustrates to us that injuries are much more random and complex than the average NFL fan understands. Lastly, QBs 28 and older missed 1.7 games on average compared to 1.5 for the younger QBs. This also tracks as the the best predictor of future injury is previous injury and as every NFL player ages, they’re nearly guaranteed to have something pop.

The data suggests that the frequency of designed runs per game does not significantly impact the number of missed games for quarterbacks. It implies that the act of intentionally running the ball does not inherently increase injury risk.
Scrambles vs. Missed Games Due to Injury
The findings imply that the frequency of scrambles does not have a considerable impact on the number of games missed due to injury. In other words, scrambling does not necessarily make quarterbacks more prone to getting hurt.
Sacks/Hits vs. Missed Games Due to Injury
The data suggests that the more sacks plus hits a quarterback sustains, the higher the likelihood of missing games due to injury. It's crucial to note that this correlation does not solely apply to rushing quarterbacks, but encompasses all quarterbacks.
All Hits vs. Missed Games Due to Injury
Examining all hits endured by quarterbacks, whether from designed runs, scrambles, or sacks, there’s a significant, small positive correlation with missed games. This signifies that all types of hits can contribute to the likelihood of quarterbacks missing games due to injuries. Once again, this correlation is not exclusive to rushing quarterbacks but encompasses the position as a whole.
Placement on the Injured Reserve
Similarly when it comes to injured reserve placement, we find that the most closely associated relationships are “sacks plus hits” and all hits.
Theory and What it all Means
As expected, when taking into account sacks + hits in pocket + scrambles + designed runs, injury rates go up. As you’ll remember, the thesis was never that taking hits is safe. However, the data does suggest that of these 4 possible outcomes for quarterbacks, taking a hit in the pocket and/or being sacked is more tightly linked to injury than any combination of a scramble and/or designed run.
Most of the risk (62%, in fact) for QB injury seems to come while standing in the pocket and taking a shot. This makes sense as players who scramble and run on a designed attempt have at least some semblance of control over whether or not they take on contact. That’s juxtaposed to a sitting duck in the pocket.
Of course, there are always exceptions and the takeaway here isn’t that all quarterbacks should scramble. Take Jimmy Garoppolo as an example. In 2022, he was initially hit in the pocket and technically “scrambled” his way into a season-ending foot injury outside of the pocket. Similarly, “designed attempt” doesn’t inherently mean that contact is the culprit of injury, as exemplified by Kyler Murray in 2022. Lastly, being in the “mobile” category does not mean sacks and hits leading to injury won’t happen (see 2022 Lamar Jackson, injured in the pocket). As always, it’s much, much more complicated than this. The only thing we can be relatively certain about is that hits + sacks lead to higher injury rates for NFL QBs.
Enter questions about Anthony Richardson.
Both injuries Ant sustained were on designed runs. Should he take on contact at the goal line and put himself at risk for a concussion? No. Was his shoulder injury a complete fluke? Yes.
The expectation is that the coaching staff and management have had that conversation and I expect Richardson to be more selective with his rushing and sliding in 2024.
Conclusion
In summary, the myth that rushing quarterbacks are more prone to injury doesn’t seem to hold up to statistical testing.
This data indicates that designed runs and scrambles have minimal influence on missed games due to injury, while quarterbacks who face a high number of sacks and hits face a significant correlation with increased injury risk.
For fantasy football players, it is crucial to consider a quarterback's ability to avoid hits, their offensive line's performance, and their overall health when assessing injury risk.
Rushing ability alone should not be the sole determining factor when evaluating a quarterback's durability. By understanding the nuances of injuries, fantasy football enthusiasts and NFL fans alike can make informed decisions when drafting quarterbacks and ultimately increase their chances of success.
Foot and Ankle Fractures
Wide Receivers
Foot/ankle fractures commonly occur in WRs who are in the first 3 years of their career
Unless a player is already on the last leg of his career and then sustains a Lisfranc, these injuries tend to be a blip on the radar
There is a higher chance generally of sprains and re-injury in future seasons, but for top players it’s not a worthwhile concern unless splitting hairs
Fibula Fracture:
These fractures can be associated with a high ankle sprain
The timeframe to return is variable, ranging anywhere from 75 days to 123 days
75% of WRs miss at least 12 weeks

Jones fracture:
A fracture of the 5th metatarsal (pinky toe side)
The timeframes to return are highly variable
There are studies to show all NFL players returning from a Jones fracture were at risk for reinjury if returning prior to 10 weeks i.e. 2015 Dez Bryant
Despite this fact, a small scale study with offensive, defensive and non-skill players found that the overall average return time is approximately 9 weeks
For WRs the return from a Jones fracture is usually between 12-16 weeks
The first season after surgery fantasy points per game normalizes to pre-injury levels
“Lisfranc” is a very broad term that refers to injuries in the middle of the foot with a whole host of subtypes which can include ligament tears, foot fractures, and joint dislocations
There are many degrees of severity and involvement
Recovery typically takes skill players as little as 6-7 months (less common) but can take up to 11-12 months i.e. 2023 Rashod Bateman
A Lisfranc injury does not always require surgery i.e. 2020 Joe Mixon

The sample sizes are not big enough to draw conclusions and should be analyzed on a case-to-case basis. Here are boxes to check when analyzing these players:
Young age (25 years or younger)
A freak athlete/good player
Good front office support with on-field opportunity
At least an average amount of time to rehab without setbacks
Ideally the player checks all or most of these boxes.
The following profiles are specifically in reference to a player’s injury and/or performance risk. They are not meant to be used as a sole determination of rankings. Pair these profiles with your favorite fantasy analyst’s tiers, rankings and analysis. Below is the key to risk assignments.
So there seems to be mass hysteria around Joe Burrow’s latest hand injury. There’s always risk when it comes to any surgery – sure. There’s risk of complications, persistent swelling and a ton of other secondary issues. But the bottom line is that an overwhelming number of athletes with this injury bounce back and do fine. There’s evidence here, too.
If there are any concerns at all, it’s really about longevity for Burrow, who has now dealt with a major ACL injury, a significant calf issue that could implicate his Achilles at some point and now this hand surgery. More specifically, there’s an increased risk of arthritis in his hand and knee now compared to when he entered the league.
Still, ironmen like Aaron Rodgers and even Cam Newton to a certain degree overcame significant injury histories. Putting talent and fantasy football relevance aside for a minute, even Carson Wentz had a rough go of it in terms of bad injury luck, but before being benched survived an entire season. Need more examples? Guys like Donovan McNabb played 13 full seasons despite a crazy injury history. The point I’m making here is that in 2024, there’s minimal reason to fade Burrow or his weapons. In dynasty? You might consider moving on from Burrow 1-2 seasons before you see a cliff – because the cliff might be steep.
Bottom Line: Risk: Slightly Above Average · 2024 Outlook: His ADP is fine
Big FantasyPoints wanted the first QB profile to be about Vice President candidate Aaron Rodgers but I refused and instead am writing about my favorite 2024 bounce back candidate – Anthony Richardson.
Listen, I don’t need to regale you with the fact that in his three healthy games in 2023, Richardson averaged 23 fantasy points which would have been good for QB2 on the year. No quarterback averaged more fantasy points scored per dropback. I don’t need to bring up the fact that he was on pace to rush for almost 750 yards. There’s no need at all to bring up that he probably would have realistically scored somewhere in the neighborhood between 10 and 15 rushing touchdowns.
Stop drooling. I said there’s no need to picture that.
Let’s start with history. Yes, Richardson injured his right AC joint as a senior in high school, ending his 2019 season before he went to UF. Yes, this injury could have compromised the strength of his shoulder four years later and contributed to his current AC joint fracture. And yes, there’s some risk of reinjury. However, AC joint sprains in particular are one of the most common reported QB injuries. Richardson’s injury was one of the most severe cases.
Now, some might point to Aaron Rodgers and Tony Romo as QBs with AC joint sprains that led to extended missed time. The problem is those injuries were to non-throwing arms. There are no great comps for Anthony Richardson for that reason. Still, players like Dak Prescott have played through the lesser version of this injury before.
As long as Richardson rehabs properly, his upside is worth any potential downside. The risk of arthritis is lower compared to injuries to the inside of the shoulder joint like a labrum (think early career Drew Brees).
If you’re worried about injuries in general for Ant, just take a stroll down memory lane by searching Aaron Rodgers’ injury history. Or Carson Wentz’ injury history (ignore the fact that he was the picture of health right before he imploded between the ears). Or the fact that Cam Newton’s career-altering shoulder injury actually came while trying to make a tackle. NFL players, even QBs, get injured. So the default setting is that they’re not injury prone until they are.
As for running QBs and injury rates, read this piece if you’re worried. The primary concern for Richardson isn’t whether or not he should be escaping the pocket. The primary concern is what he does once he leaves the pocket. If he can learn to protect his body more like Lamar Jackson, Richardson is in for one of the best fantasy careers we’ve seen for a QB. The upside is absolutely worth any risk he brings.
Bottom Line: Risk: Slightly Above Average · 2024 Outlook: Worth all risk involved because of his ceiling
Captain Kirk tore his right Achilles tendon and had surgery to repair it in November. Since then he’s been on Dancing With the Stars, making Week 1 all but a lock. But the problem was never whether he’ll return – it's what he will look like once he is back. That and the re-injury rate, which is generally less than 5%.
It's important to note that this is Cousins’ plant leg while throwing, meaning he will be placing more stress through the repaired tendon than Aaron Rodgers, who injured his left Achilles. So what are the takeaways?
Realistically the biggest risk is that the 35-year-old Cousins has recurring calf strains throughout the season and has to sit 2-3 weeks. Or he plays through it the way Burrow did at the start of 2023. The best (and most likely) scenario is he’s now even less mobile than before and checks down to Bijan Robinson early and often. So draft Bijan. Kirk is fine.
Bottom Line: Risk: Significantly Above Average · 2024 Outlook: Decent value. Draft his skill players.
Rodgers’ situation isn’t much different than Cousins’. They’re both at risk for calf strains as older QBs, but this shouldn’t impact their teammates’ performance much. The fact that this isn’t Rodgers’ plant leg is a wash considering he’s 5 years older than Kirk. Draft Breece Hall and Garrett Wilson.
Bottom Line: Risk: Significantly Above Average · 2024 Outlook: Draft his skill players.
Much talk has been made about Joe Burrow’s unprecedented wrist injury but Deshaun Watson has also entered uncharted waters. His shoulder injury was a strange one that required fixation of the fracture. This injury is vastly difference Than Anthony Richardson’s as the portion of bone that was fractured sits inside the joint. For those following a while you know that injury inside of any joint immediately raises the risk of early onset arthritis.
The closest comp for this injury is Drew Brees’ 2005 labrum repair. Obviously Brees had a hall of fame career but he admitted himself if it weren’t for his shoulder giving out, he could have played longer. That’s the primary risk for Watson is a shortened career as opposed to immediately looking like a noodle arm. He should be back by Week 1 as long as there are no reports of a setback. Given his ADP, taking a handful of Watson is…fine.
Bottom Line: Risk: Slightly Above Average - Significantly Above Average · 2024 Outlook: Should be fine in 2024
Hebert suffered a “plantar fascia” injury and will reportedly be in a boot for 2 weeks. He’ll bump right up Week 1. This is a similar injury to Tony Pollard from 2021. That year Pollard missed one game but returned shortly. Herbert can play through this in 2024 but the rushing upside is truly capped at this point. His ADP is low but given the way this offense is shaping up (along with Herbert missing valuable camp reps) its not likely Herbert shows any real ceiling this year.

Brooks had isolated ACL surgery on Dec. 5, 2023, making his estimated 9-month return right around opening day of the 2024 NFL season. However, given the reported nature of his injury and his age, it would not be shocking to see Brooks on an NFL field by Week 2 or 3. Now this goes back to what teams should do vs. what they will do. If the Panthers want to give their 2nd-round draft pick adequate time to get right, they can wait to bring him back in Week 5 at the latest.
In terms of production, there aren’t great comps for a rookie RB coming off of ACL rehab. So, I want to tread lightly – but Todd Gurley is the best we got. Gurley had surgery around the same time in the college season as Brooks. Then after sitting the first 2 games of the 2015 season and seeing just 28% of snaps in his first game back, Gurley bounced back to win Offensive Rookie of the Year finishing as RB8 on FPG basis. Keep that finish in mind.
That year the Rams were middle of the pack in scoring and in expected points added via rushing. Furthermore, after ACL surgery most RBs see a dip in Year One efficiency. So what helped keep Gurley afloat? Ten rushing touchdowns and a healthy 3 fantasy points added via receiving work per game. This is crucial to bouncing back after an ACL tear because efficiency takes a major dip. Guys like Adrian Peterson, Breece Hall and Gurley himself all saw a healthy amount of passing game work.
An even more ironic statistic is that together in 2023, Miles Sanders and Chuba Hubbard combined to average 16.2 FPG. In 2023, that would have been – wait for it – RB8.
Moving on to the criteria for successful post-ACL RBs. We’ll complete this exercise with Nick Chubb later but here’s how Brooks stands up to the Breece Hall test:
Minimum of 9 months to rehab - yay!
High NFL draft capital - yay!
26 year or younger - yay!
Minimal complexity of injury - yay!
Top 5% athleticism scores - close but boo
Projected pass catching work and high touch volume - boo again (at worst)
So the question is simple: Can the Panthers be at least mediocre in 2024 after finishing dead last in total offense and 20th in rushing offense in 2023? Will they give enough receiving work over to Jonathon Brooks to boost production? Brooks isn’t likely to be this year’s Breece Hall. He may not even match Todd Gurley’s rookie year. What Brooks is likely to provide is solid late-season production and easily pay off his RB28 ADP.
Bottom Line: Risk: Slightly Above Average · 2024 Outlook: Even in a bad offense, could be a league winner
Chubb suffered a multi-ligament left knee injury in Week 2, 2023 that required two surgeries. Both of these surgeries and his subsequent recovery are crucial to understand. The first surgery was 9/29/23. However, the time-limiting factor is primarily the ACL surgery, which was performed on 11/14/23. It is not uncommon to wait between surgeries, though it isn’t necessarily a good thing.
Dynasty managers, this is especially important: You recall that Nick Chubb tore everything except his ACL and MCL in his left knee while in college in 2015. Now, he suffered another dislocation with additional damage in the same knee. The hill that Chubb has to climb just to simply get back in shape and play again at 28 years old is one thing. Fending off early-onset arthritis after two massive injuries in the same knee is another. From a dynasty perspective, I’d offload Chubb ASAP while he still has some perceived value. Because it could easily get worse. Think Todd Gurley worse.
For re-draft managers, the date of the ACL surgery is key. For first-time isolated ACL injuries in guys nearly a decade younger than Chubb, nine months is the bare minimum rehab period before full clearance to return to practice. In Chubb’s case, that date is 8/14/24, which is three weeks into training camp and the middle of preseason. With his 2015 knee injury in the past, additional damage (ACL), and the potential for complications from the first surgery (stiffness/swelling) – it's extremely difficult to see the Browns bringing him back that quickly.
Chubb has been an iron-man, missing just seven games in his first five seasons. Unfortunately, he only meets a few traits to be considered an outlier performer post-ACL surgery:

High NFL draft capital - yay!
26 year or younger - boo
Minimal complexity of injury - double boo
Top 5% athleticism scores - close but triple boo
Projected pass catching work and high touch volume - boo again
Look, there are simply some guys who are truly built differently, and Chubb is one of those guys. For that reason, I won’t speak in absolutes and give Chubb an arbitrary 5-10% chance to come back and return as a back-end fantasy RB1. Those might seem like long odds, but 30% of NFL RBs never return after an ACL tear.
At best, we’ll see Chubb begin taking contact towards the end of camp in mid- to late-August and be ready to return to the field by the end of September. If Chubb has no complications, his ceiling is slightly higher than Javonte Williams last season (11.2 FPG – RB31). There’s a good chance that he starts the season on the PUP list and misses the first four games of the season at minimum. Lastly, the average RB post ACL in year one sees a 12% dip in fantasy production. Shaving that off of Chubb’s last 3 seasons would have made him Josh Jacobs (RB18) in 2023 - not exactly a league winner. Crazy heavy squat be damned.
Bottom Line: Risk: Significantly Above Average - Extreme Risk · 2024 Outlook: He’s built different but faces an uphill battle
For a minute let’s put J.K. Dobbins’ career-altering ACL+ injury aside. Let’s discuss his Achilles injury in a vacuum. Here are Dobbins’ pros and cons about returning to play and performance metrics post Achilles rupture.
Pros list:
Relative advancements in tendon rehab
Younger age
High NFL draft capital
Crazy pre-injury athleticism (speed specifically)
Cons list:
72% return to sport rate in NFL players after an Achilles repair
50% reduction in “power rating” (imperfect metric but the point remains)
RBs & LBs have the worst outcomes
The pathophysiology of the injury
The last one is important and remains the reason why ACL and Achilles injuries are vastly different. See, the achilles is an extension of the calf muscles. The calf muscles transmit force to the foot and ankle via the Achilles tendon. When the tendon is repaired and remodeled, it’s never the same.
We’ve been through this with Cam Akers’ hasty return from this injury — which was truly incredible in its own right and shouldn’t be ignored — but even then, Akers’ performance was…fine. Now he’s out of a starting job. The bottom line is this injury has yet to completely shed the career-killer reputation. This will be an uphill battle for Dobbins and the chances of him returning and being a major factor this season are slim to none. At his current ADP there’s very little risk. But based on history, there’s very little upside either. In 100 drafts I’ll take Dobbins 5 times max.
Bottom Line: Risk: Significantly Above Average - Extreme Risk · 2024 Outlook: Exposure to Dobbins comes with a limited ceiling and rock bottom floor
There was some optimism on Ekeler initially until I discussed this with Ryan Heath of fantasypoints. See, there’s a decent chance that Ekeler – the dude who rushed for 116 yards and caught 4 balls in Week 1 of 2023 - is legitimately washed. Even though there’s a small chance Ekeler played on a minor fracture in 2023, it's a fact that in year 8 RBs average 73% of career production. Sure enough, Ekeler is walking into a presumably below average offense competing against a younger and capable backfield mate with a rushing rookie QB. Even if Ekeler isn’t totally washed, would it be surprising to see a major dip in touches and efficiency? Not a chance. Unless Brian Robinson Jr. is injured, Ekeler will likely be an ADP beater at best but has a limited ceiling.
Bottom Line: Risk: Significantly Above Average · 2024 Outlook: Father time comes for us all
I was totally out on Javonte last year as 2023 Injury Prone Draft Guide purchasers are aware. Well, I’m back in baby. Admittedly his ADP was scary low to start the offseason but has steadily increased which I think is correct. What has really helped is Sean Payton making positive comments about the Denver running back once per month (literally) since June. Listen, nobody is trying to tell you the Denver offense will be potent this year but given the high rate of RB targets in Payton-run offenses along with Bo Nix’s own propensity to check the ball down, Javonte could be in for more volume than most of us expect.
Oh, right. The ACL.
It's worth noting that Williams’ complex injury was never going to turn out good in 2023. However you should know that RBs who survive into year 2 after an ACL tear fare much better.
NFL RBs who survived to year 2 post ACL improved in:
*Missed tackles forced (+17%)
*Yards created (+9%)
*Receptions (+39%)
*Fantasy points per game (+132%(!))
Even if Williams sees a 10-15% degradation in skill (which is not necessarily expected) his outlook at RB33 is below his floor. Williams is one of my favorite RBs to target when I’m going anchor RB. Lastly, here’s a list of RBs who improved by at least 50% in fantasy PPG from year 1 post ACL to year 2 post ACL:

Is Javonte at least Gus Edwards or Rashaad Penny? I think so. Here’s a last tidbit for you: despite it all, Javonte was actually 7% better in 2023 compared to 2022 from a fantasy PPG basis.

Phew I owe the entire Allen family an apology. Here’s what I said in last year’s Draft Guide.
“For this first time in his career I would consider Allen sneaky risky as opposed to generally safe”
So what did Allen do? Shoved it directly down my pie hole finishing as WR3 on a PPG basis in half PPR missing no time due to the hamstring. This was of course also a career year for the former Charger. In my defense, there was no real way to predict the outlier outcome and we can’t simply play the result.
So here we are again with a 32 year old Allen. Now, will Allen provide another WR3 season? Even if he’s healthy, that’s not likely. Can he play in 14-16 games and provide top 15-ish value? Sure. Just remember that a soft tissue recurrence is still at play and he’s a high risk/moderate reward type player in 2024.

In 2021 Cooper Kupp averaged 21.6 half PPR fantasy points per game. It remains the highest mark in 9 seasons for a WR. He followed that up in 2022 by averaging 18.2 half PPR points and finished as the WR1 in PPG - again.
In 2022 Kupp broke his ankle but was still - rightfully so - a top 10 pick by ADP come summertime. Then what happened?

There’s no denying it - the context of the Rams offense has changed by adding Puka who has stepped into the downfield role. But how exactly has that impacted Kupp? Let’s compare Kupp’s 2022 numbers to the last 4 weeks of his 2023:

Team target share: 24.6%

Cooper Kupp in 2022:

Team target share: 30%

The obvious impact Puka has made on Kupp is in the air yards share and yards per route run. So even though Puka’s target share in the final weeks of the season was 23.5% (compared to Kupp’s 25%) Puka’s targets are worth more.
Kupp isn’t washed. In fact, he’s still top 6 in target share amongst qualified WRs according to FantasyPoints Data.
The next point to bring up is the 33% recurrence rate in hamstring injuries and the fact that the older a player is, the more likely a recurrence is to happen. If only there was a 31 year old previously top WR we tried to write off as washed because of a severe hamstring (Keenan Allen). Turns out he overcame the recurrence rate and finished as a WR3.
Lastly, here are the last two years of teammate WRs who have finished inside the top 25 on a PPG basis:

Be honest: how surprised would you be if the Rams top wideouts both finished as top 10 WRs with Sean McVay in control? Sure, there's a risk Kupp falls off or if re-injured. But the truth is that the median outcome is Kupp finishes as a top 15 WR. And even if it isn’t the most likely outcome, he can still finish inside the top 5. He’s worth any perceived risk.

We’ve all seen (EP note: This hot link is to an Achilles rerupture rate article) #ThighGapGate on the internet and even though some managers are taking this to the bank, there are a few details to tease out. These are best demonstrated as a set of questions:
Was the gap in Christian Watson’s leg sizes present before his injuries or did 2 years of injuries lead to the gap?
In a sport that is largely asymmetrical, does the size difference really matter?
Did the Green Bay medical staff already try to correct it?
How many athletes have attempted to correct an “asymmetry” actually showed an observable difference? Alvin Kamara would like a word here
The point: it's great to hear Watson and his team are trying to resolve a recurring problem. But what are the facts?
Previous injury is the best predictor of future injury – specifically when it comes to soft tissue. What's worse, a high density of type II muscle fibers (“quick twitch” fibers) are a risk factor for hamstring injuries in some athletes. So yes, that sub 4.4 40 could be hampering Watson’s full recovery.
Since 2018, the highest reinjury rate from year to year is – you guessed it – hamstring strains in WRs. Odell Beckham and Deebo Samuel are players who in Year 1 and 2 struggled with hammy issues. However, OBJ didn’t miss time in Year 2 and Deebo’s body type and physical demands are vastly different from Watson’s. That leaves us with the legend of Isaac Bruce, which again is imperfect as he fought off hamstring strains in Year 3 and 4.
The last in line to buck the trend: Will Fuller. Strangely Fuller missed zero games due to injury in college. In NFL Years 1-3, he missed multiple games due to a hamstring injury. It seems the problem compounded instead of improved. This followed him into the 2019 season, meaning Fuller had nearly a calendar year to get his hammies right while rehabbing the ACL but no dice.
Will Fuller also took extreme measures to get himself right but never could. This is, of course, a shame as he was electric when available.
The takeaway: Christian Watson’s floor, plain and simple, is Will Fuller. Still, it's hard to shake the fact that in his last 2 regular-season games Watson scored 20 and 27 PPR points good for WR8. The WRs in Green Bay are all highly talented and Watson will likely need consistent volume in addition to good injury luck to hit top-15 value. Still, at his current ADP in the 7th round, it’s not hard to click his name and give him one more chance in 2024.
Bottom Line: Risk: Significantly Above Average · 2024 Outlook: Everyone deserves second chances – especially at this ADP
Williams gets a bad rap for missing time due to injury, but the narrative is overblown. From 2018 to 2022, the wanna-be WWE Superstar has been catching contested balls and getting swiftly Rock Bottom-ed to the ground in 16-15-15-16 and 13 games. In other words before the ACL, he missed just under 2 games per season. That’s beyond the point.
Now, he 29-year-old is fighting to bounce back – but he’s facing an uphill battle. Why? Because 63% of WRs see at least a 13% production dip in year 1 post-ACL. And the names on the list of guys who were successful, well. They don’t exactly look or play like Mike Williams does:
Courtland Sutton
Jordy Nelson
Allen Robinson
Cooper Kupp
Julian Edelman
Jeremy Maclin
A ceiling year for Mike Williams would be something like 11 half PPR points per game, which would certainly beat his WR60 price tag. Still, that translates to approximately WR20 to WR28. Here are some WRs I’d rather take a chance on that go around Mike Williams’ current ADP:

Josh Downs
Ja'Lynn Polk
Jahan Dotson
Xavier Legette
Adonai Mitchell
Williams is an OK pick if you’re looking for some sort of floor. But we draft for the ceiling around here.

On Nov. 16, 2023, Mark Andrews was ruled out for the remainder of the regular season with a fractured fibula. As noted, he attempted to return by the AFC Championship but was predictably limited. Now, he’s had the entire offseason to rehab and recover. Barring abnormal complications, Andrews should have no problems returning by Week 1 in 2024.
The most notable comp for a top TE coming back from this type of injury is TJ Hockenson from 2020. He followed up his 2019 ankle fracture with a cool 67/723/6 receiving (on 101 targets) while playing in all 16 games. That was good for TE7 in 2020 on a FPG basis, which is likely the floor for Mark Andrews. His current ADP is TE4 at pick 49 based on Underdog – meaning it might be even lower in home leagues. He’s 14 picks after Sam LaPorta and behind Trey McBride. I would get as much Andrews as possible before gamers come to their senses.
Bottom Line: Risk: Average Risk · 2024 Outlook: ADP is great
The Vikings TE had surgery to repair his torn ACL on January 29th of this year. Hockenson also injured his MCL, but only had his ACL operated on. Hockenson admitted that the Vikings “haven’t really put a timeline” on his recovery. This makes sense with the established 9-month rule of thumb for ACL returns being considered.
When looking at wide receivers who had ACL surgery in January or February since 2013 – a less physical position which implies the ability to return faster than a TE – the results aren’t promising:
Paul Richardson - November return
Devin Smith - November return
Eli Rogers - November return
Chris Godwin - September return. Then injured his hamstring (October return).
Michael Gallup - October return
Odell Beckham Jr. – Missed Weeks 1-2, returned Week 3 late-September
In terms of tight ends who are a similar archetype to Hockenson, the sample is small but does something like this:
Rob Gronkowski - January surgery and Week 1 return
Zach Ertz - November surgery and Week 1 return
Logan Thomas - December surgery and Week 1 return. Then 4 week calf injury
None of these TEs had surgery as late as Hockenson. Some might argue that Gronk’s 8 month return sets the precedent here. The problem in Hockenson’s case is that, even at an accelerated 8-month return, it still leaves plenty of room to start him on the PUP and not play until Week 5 in October. Even though Gronk returned that quickly, he wasn’t able to handle his usual workload. Gronkowski was capped at 60% of the snaps until Week 5 of the 2014 season. Aside from timing, his size – 6’5” and 250 pounds – is working against Hockenson. It could take big boys longer to truly get their feet under them.
Gronkowski, Ertz, and Thomas all saw their efficiency drop in Year 1 post-ACL surgery. Gronk’s yards per target dipped by 13.1% to a career-low at that point (9.9 career YPT to 8.3 in 2014). Ertz and Thomas were both on the wrong side of 30 when they sustained their injuries, and both suffered big dips in efficiency measured by YPT. In Ertz’s case, he averaged a pathetic 4.3 YPT last season in his first year off his torn-ACL. That’s a 39.5% drop compared to his career baseline (7.1 YPT). Thomas’ efficiency dropped by 16% in 2022 post-ACL compared to his career average. Luckily, Hockenson does have youth on his side. He just turned 27.
Hockenson had 95 receptions and finished as the TE2 in fantasy points per game with nearly 1,000 receiving yards in 2023. It was his fourth-straight season as a top-8 scoring TE by FPG. He has returned well from injury in the past, following up his 2019 ankle fracture with a cool 67/723/6 receiving (on 101 targets) while playing in all 16 games on a worse Lions offense in 2020. Still, he’s three years older now, and it's hard to take a flier on him in best ball considering gamers will likely have to take at least 4-5 zeroes from him to start the season. A dip in efficiency should be expected in his first year coming back from injury, too. Expect a Week 7-8 return near Halloween (or later). He’s an I.R. stash-only for seasonal leagues.
Bottom Line: Risk: Significantly Above Average · 2024 Outlook: IR stash at best, could pay off in the playoffs
Kittle finally underwent a procedure to correct a recurrent groin strain this offseason.This is (presumably) the same procedure Rashod Batemon had done in 2021. Athletes who undergo this surgery continue performing at a high level but play fewer games per season (12 vs. 14) and last a shorter time in the NFL (3 years vs. 4 years). Realistically for a freak like Kittle, maybe he still misses 1-2 games due to the groin this year but his career isn't likely in jeopardy. This ADP is fair.

Here is a list of common injuries with medical definitions (in layman’s terms) if you’re so inclined. This is by no means an exhaustive list as that could be a book on its own (spoiler it’s called PT or medical school).
Concussion AKA TBI
The objective definition of a concussion is a traumatic brain injury
This occurs when the head and brain are moved violently back and forth leading to chemical changes at times damaging brain cells
Every concussion predisposes a person to a subsequent concussion
These injuries are dangerous and some of the least predictable in all of sports medicine
Hamstring Strain AKA “Soft Tissue Strain”
A group of 3 large muscles in the back of the thigh that bend the knee, help deceleration and extend the hip
These injuries fall into the class of injuries the fantasy community generally refer to as “soft tissue” injuries
Hamstring injuries cause a large variation in missed time and tend to recur, causing objective dips in production with a boom/bust nature once the player returns
The 1st week back from injury is the riskiest time to insert these players into a lineup and challenges the “start your studs” argument.
Osteoarthritis is a term for an ongoing process that degrades the cartilage within the knee joint but can occur in any joint in the body
The body constantly tells the brain to release a cascade of chemicals to the area and "clear up" an injured body part. These same chemicals signal the brain that there may be damage and the brain can interpret the stimulus as “painful”
O.A. never ends. Due to the constant signaling of damage, the cycle continues to repeat itself.
Lateral Ankle Sprain AKA “Low” Ankle Sprain
A lateral sprain occurs when a player forcefully “rolls'' onto the outside of his ankle/foot.
Ankle sprains are an extremely common NFL injury, accounting for 12% of all injuries from 2012-2014
By the time players reach the NFL combine, 13% of running backs and wide receivers and 7% of tight ends have already had some sort of ankle sprain
Players tend to play through lateral ankle sprains, but they can be catastrophic as in the case of Saquon Barkley in 2021 and Jonathan Taylor in 2022
Syndesmosis Sprain and/or “High” Ankle Sprain
This tissue is damaged when a player is forcefully compressed while the lower leg is rotated
These injuries are consistently more debilitating than lateral sprains because the syndesmosis has a poor blood supply and high sprains typically involve a greater volume of tissue damage
Labrum Tear AKA “Shoulder Dislocation”
The best example of a player pushing through several labrum tears is Dalvin Cook over the last few seasons
Some of those games were at critical points in the fantasy season, like in the 2019 fantasy playoffs
These injuries have an extremely high recurrence rate
Shoulder A.C. Joint Sprain AKA “Shoulder Separation” or “AC sprain”
A very common injury to QBs, AC sprains occur when a player is tackled and taken down forcefully onto the side of his shoulder
They typically cost players 9.8 days per injury
However, quarterbacks miss the most time when severe enough, missing an average of 17 days due to the injury
Recently Deebo Samuel and Ja’Marr Chase had this injury in 2023
Timeframe specific to grade II AC sprains: Approximately 2-4 weeks (highly variable depending on positional demands)
MCL sprain
The primary function of the MCL is to resist the knee from caving inward
Logically, this ligament is over-stressed when the knee is forced into valgus and this study on soccer players confirms that 70% of all MCL injuries are from forceful valgus that is contact in nature
It is not uncommon for a high-ankle type injury to be confused on video with an MCL sprain as they can occur together
MCL sprains account for 41% of all NFL knee injuries, but the injury is rarely season-ending unless coupled with additional damage.
Recently De’Von Achane suffered this injury
Timeframe specific to grade II MCL sprains: Approximately 4-5 weeks
PCL sprain
The PCL is the sister ligament to the ACL and prevents the knee from being forced backwards (among other things)
Recently Zay Jones and Gave Davis have sustained this injury
Timeframe specific to grade II PCL sprains: Approximately 2-4 weeks but highly variable (NCAA offensive players)
Sports hernias are muscular strains that may be listed as “hip,” “groin” or “abdomen,” and at times may even be listed as “osteitis pubis.”
Severe sports hernias will initially be treated with 4-6 weeks of physical therapy. If this does not solve the issue, players will undergo surgery that will take a recovery time of 58 days if done in any month other than December.
The biggest recurrence for a sports hernia is a previous one. If a player has surgery to correct the problem, they’ll continue performing at a high level but play fewer games per season (12 vs. 14) and last a shorter time in the NFL (3 years vs. 4 years)
Christian Kirk is coming off of this surgery - which is generally successful.
These studies used the official practice reports of all 32 NFL teams from the 2018 regular season through the 2023? regular season. Players who were on the injury report during that time were identified to calculate performance metrics in the first week back from the most common injuries at the position and/or playing through an official designation on the injury report with that injury. Because this study is meant to analyze players who are fantasy relevant, points per game cutoffs (in PPR scoring) were used at every position listed in the definition tables below. Sample sizes were taken into consideration and if there were not enough players returning after missing time, all players who played throughout the designation but did not miss time were included. This was only done for RB concussions and for all injuries at the QB position. This study is far from perfect and is designed to be a general guide for gamers and sports bettors to use in conjunction with player-specific injury analysis. Although I’m still (mainly) a one-man research team, the vision is to continue expanding this guide for years to come and perfect it

Lastly, I really wasn’t expecting to be this low on Pitts but I’ll say this: I think he’ll start slow and can be fine. The decision to take Pitts will lie in your risk tolerance as a hamstring and PCL repair (potentially more structures were involved) will loom large.
